• Certified photocopy of Official Transcript of Records
• Transfer Credential from last school attended
• Original NSO (birth certificate if single,marriage contract if married)
• Two pieces of 2” x 2” latest ID picture

2. For Diploma Courses

• Degree/work related to the program
• Duly accomplished application forms
• Transfer Credential from last school attended
• Two pieces of 2” x 2” latest ID picture

3. For Certificate Courses

• Duly accomplished application forms
• Two pieces of 2” x 2” latest ID picture

4. For Short Courses

• Duly accomplished application forms
• Two pieces of 2” x 2” latest ID picture
