Graduate Program:

• Official Transcript of Records
• Transfer Credentials or Honorable Dismissal
• Accomplished Application Form
• NSO Marriage Contract (married women using husband's family name)
• Two latest colored photo ID (2" x 2")

Undergraduate Program:

• Form 138 (new freshmen)
• Transfer Credentials or Honorable Dismissal (tranferee)
• Certification of Good Moral Character from previous school
• Accomplished Application Form (available in website)
• NSO Birth Certificate
• NSO Marriage Contract (married women using husband's family name)
• Two lates colored photo ID (2" x 2")

Foreign Students (additional requirements):

• Student Visa
• Official Transcript of Records in English
• Notice of Acceptance (NOA)
• Alien Certificate of Registration (ACR)
• Result of TOEFL where English is not the medium of instruction
• Two Latest colored photo ID (2" x 2")
• Other requirements by the Bureau of Immigration

* Notice of Admission (NOA) - Given to incoming students who have taken and passed the entrance examination.
